Family Hospice Care. When person is told they have six months or less to live and with a doctors order they sign on to hospice. We help them physically, mentally, spiritually and emotionally through the dying process. A team of professionals are available for family and patient: Doctor, Nurse, Social Worker, Spiritual Counselor, Home Health Aide, Bereavement Counselor & Volunteer. Family Hospice Care goes wherever the patient resides - be it their Home, an Assisted Living Facility, a Board and Care or a Skilled Nursing Home. Hospice makes the dying process more comfortable as RNs are pain specialists. Social Workers help families get paperwork in order and assist with resources, Home Health Aides assist with personal hygiene, Spiritual Counselors assist with any spiritual needs and volunteers offer socialization, caregiver relief and shopping among other things. Pre bereavement is offered. Family Hospice Care has an outstanding and dedicated group of individuals from management down who work together for what is best for patient and family. Family Hospice Care is teamwork at its finest with the end goal being a peaceful death.
